The parenthetic ‘American’ is unnecessary. I wouldn’t go so far as to say that they don’t exist elsewhere but the only places where I have come across non-royals who use ‘junior’, ‘III’ and so on are the USA and US territories and ex-colonies, such as the Philippines.
This usage is almost completely unknown outside of the USA and is an immediate indication that the person is almost certainly an American. It is also a pretty good sign, like the double-barrelled name in the UK, of a probable, strong streak of pomposity.
